Final Requirements

1.   5.03(a) You must provide a copy of your current valid driver's license 

2   5.03(c) You must give permission for the employer to obtain your Motor Vehicle Record - This will be performed annually 

3.  5.03(d) You must complete the Certificate of Receipt found in the addendum of the current CDE Type A, Multi-function and small Vehicle Route and Activity Operator  Guide found at this link http://www.cde.state.co.us/transportation  under the heading CDE Guides. A copy must be maintained in your driver qualification file at the district.

4.   5.03(e) You must successfully pass the current year Type A, Multi-function, Small Vehicle Route/Activity Operator Written Test with a score of 80% or above, and a copy must be maintained in your driver qualification file at the district.

5.   5.03(f) You must complete the current year STU-17 CDE Multi-function/Small Vehicle Operators Medical Information Form found at this link https://www.cde.state.co.us/transportation/stu17vehicleoperatormedicalinformationform and a copy must be maintained in your driver qualification file at the district.  

6.   5.03(h)You must provide written documentation that you have received First Aid Information and a copy must be maintained in your driver qualification file at the district.

7.    5.03(i)  You must successfully pass a driving performance test including a pre-trip inspection prior to transporting students.  Written documentation shall be maintained in your driver qualification file at the district.

8.  You must provide a printed copy of the Course Completion Document provided at the conclusion of this training to your district and a copy must be maintained in your driver qualification file at the district. This serves as your documentation of PRE-SERVICE Training.
